Our Home Health staff provide compassionate, reliable, personalized in-home care to people with a wide variety of disabilities. GCI Home Health is a licensed home health agency owned and operated by Greeley Center for Independence, a trusted non-profit organization that has served the Greeley community for over 30 years.
Our medical and non-medical services have enabled clients to live as independently as possible in the comfort of their own homes. Our professional staff are dedicated to providing quality care that truly makes a difference in the lives of our clients.
Services Tailored to Unique Needs
We recognize that each person has unique support needs—no two clients are alike. Our goal is to deliver a customized set of services that meets each individual’s needs. Here’s how we do it:
We begin by listening to you to determine what kinds of services you need and want.
We work with you to create a plan of care that ensures that you can be as independent as possible in your own home.
We work with your doctor to ensure that the plan of care is medically appropriate.
We work with your case manager to ensure that the plan of care provides an approved level of service.
As your support needs change over time, we modify the plan to ensure that you receive the services you need.
Our Home Health Services
Whether you need a high level of medical care or simply need intermittent assistance with housekeeping and errands, our qualified, caring staff can help.
Private Duty Skilled Nursing
Clients receive one-to-one skilled nursing care provided by a Registered Nurse. If you use a ventilator, require tracheotomy care or have other needs requiring care by a nurse, you may qualify for this level of service.
Skilled nursing services are provided by a Registered Nurse (RN) or a Licensed Practical Nurse (LPN) under the direction of a Registered Nurse. Services include medication management, wound care, diabetic care, catheter care, injection administration and more.
Certified Nurse Aide
Licensed Certified Nurse Aides (CNA) provide skilled personal care for activities of daily living under the direction of a Registered Nurse. Services include medication assistance, bladder/bowel care, blood pressure checks, skin care, skilled bathing/dressing/oral care and more.
Personal Care and Homemaking
Trained staff members provide support with housekeeping, meal preparation, laundry, shopping, errands and more.

(Please note: Applicants are required to provide a copy of 2 (two) forms of ID with application, such as State issued Driver's License or ID card, Social Security card or passport.)
